• Patient should produce Photo ID Card, Insurance ID Card and Current Year Policy Papers.
  • Fill Pre-Authorization Form and send to TPA at least for 24 hours before Admission
  • For Planned Admission, request for Pre-Authorization after obtaining due details from treating Doctor, should forward to concerned,insurance Provider, 7 days prior to the expected date of Admission
  • For Emergency Admission, Pre-Authorization should reach TPA, within 24 hours of Admission.
  • Addition Test/Diagnosis/treatment should be approved by TPA’s.
